Personally Identifiable Information and Protected Health Information

We collect personally identifiable information (PII). This includes, but is not limited to: your name, date of birth, marital status, street address, e-mail address, credit card information, telephone number, and social media information. You do not have to provide your name or other information that personally identifies you (“PII”) in order to just view the Site.

If you choose to obtain a quote using our quote tool, you will need to provide us with information such as your birthdate, gender, zip code, and information about whether or not you smoke. After receiving the initial quote information from us, you will have the option to further customize your quote by providing us with protected health information (PHI) such as your height and weight, and responses to brief questions about tobacco use, heart health and family history.

If you choose to select an insurance product and apply for it, you will then be required to provide additional information as part of your insurance application process.

The type of PII we collect from you, and the extent to which we use it for the application and other processes, will vary depending on the product or service you selected. This may include but is not limited to:

  • Information provided in personal interviews or on applications and related forms, for example, name, address, birthdate, Social Security number and annual income.
  • Third-party reports, such as consumer credit history, motor-vehicle records, demographic, and/or medical information, if relevant to your product or service.
  • Third-party reports, such as consumer credit history, motor-vehicle records, demographic, and/or medical information, if relevant to your product or service.

Use of PII and PHI

We use your PII and PHI to fulfill your requests from us. This includes, but is not limited to, processing your insurance application, providing you with insurance quotes, and providing you with customer service.

We use your PII as we deem appropriate for our internal business purposes and as necessary to comply with our legal and regulatory obligations.

We may also share your PII with selected partners at any time.

Opting Out of Our Use of PII and PHI

In general, “opting out” means that you want us to limit the PII we collect and store about you and to not target ads that are based on your interests to your browser or device.

If you use multiple browsers, computers or mobile devices, and wish to limit the PII and NPI that we collect and store or do not wish to receive our interest-based advertising on any of them, you must opt out from each browser, computer, and mobile device individually.

Please note that if cookies are not enabled in your browser, or if you use certain ad-blocking tools, our opt-out mechanism may not work properly. Please also note that deleting your cookies does not opt you out.

If you want to opt out from our use of PHI, you need to contact us. Please refer to the Contact Us section below for more information. Opting out may impair our ability to provide you with information and services you requested.

Non-Personal Information

We collect certain non-personally identifiable information (“NPI”). This includes, but is not limited to, your IP address, browser type, click stream data, HTTP protocol elements, and status of cookies placed on your computer by the Site.

We occasionally share NPI with selected partners.

Website Visitor Tracking Tools That We Use

We use website visitor tracking technologies such as cookies, web beacons and pixels.

Cookies are text files that are stored on your computer or device when you visit some websites. We use cookies to help identify your computer or device so we can tailor your user experience by remembering the information you previously provided.

You can disable cookies already stored on your computer or device, but this may result in having to reenter information you previously entered or having some parts of the Site not work for you. To disable or modify how cookies are used on your computer, please review your web browser settings If you set ‘Do Not Track’ mode in your browser, cookies will not be used to track you. If you subsequently turn off ‘Do Not Track’ mode, you may be required to opt in again for use of cookies.

Emails we send to you may contain software code known as a “web beacon” or “pixel.” This code helps us understand the time and date that you opened an email and when you used a link within the email to visit a website, allowing us to collect analytics about our traffic, customize our services, and send you ads about our services. To disable web beacons, consult your email client program’s documentation.

You cannot remove or block web beacons as they form part of the content of the Site, however because they work in conjunction with cookies, turning off cookies will prevent web beacons from tracking your user activity on our Site. The beacon can still account for an anonymous visit, but your unique information will not be recorded.

Disclaimer

Without limiting the application of any of the foregoing, we will use and disclose your PII, PHI and/or NPI to comply with any legal obligation we have and to prevent fraud or other crime.

We will disclose or transfer PII and PHI to third parties in connection with selling or liquidating any part of our business or assets.

Site Security

No website, database or system is completely secure. We have reasonable data security and privacy practices and procedures in place to help prevent unauthorized use or access to your PII and PHI.
